Resmetirom FDA Approval Probability

What this research found

Resmetirom became the first FDA-approved treatment for NASH — now called MASH — in March 2024, after a decade in which rivals such as obeticholic acid and elafibranor failed. Working after the decision rather than ahead of it, K-Dense reconstructed why resmetirom cleared where the others did not, benchmarking Phase 3 efficacy, safety and weighted regulatory drivers before modelling the commercial opportunity. Safety turned out to be the separator: a 3.7% pruritus rate against obeticholic acid's 51.0%.

  • The pivotal MAESTRO-NASH trial met both co-primary endpoints. On 100 mg resmetirom, 29.9% of patients achieved NASH resolution against 9.7% on placebo, a 20.2 percentage point gap, and 25.9% showed at least one stage of fibrosis improvement against 14.2%, an 11.7 point gap, both at p<0.001.
  • Tolerability, not potency, decided the outcome. Resmetirom caused pruritus in 3.7% of patients against 51.0% for obeticholic acid, an LDL cholesterol rise above 10% in 2.8% against 27.8%, and adverse-event discontinuation in 5.3% against 14.0%. Obeticholic acid's advisory committee voted 12 to 2 against approval in June 2023.
  • On efficacy alone resmetirom sits mid-pack: 30% NASH resolution and 26% fibrosis improvement, against 59% and 34% for semaglutide and 62% and 51% for tirzepatide, both still in Phase 3. Its edge is oral dosing, a clean safety profile and being first to market.
  • A weighted scoring of six approval drivers gave resmetirom 40.1 points out of 48 against 25.5 for obeticholic acid, with unmet need and safety carrying the heaviest weights. The FDA approved it on 14 March 2024, four months ahead of the review deadline and without convening an advisory committee.
  • The approved label landed at the optimistic end of the pre-approval range, covering F2 to F3 patients and accepting non-invasive tests instead of mandatory biopsy. That outcome had been assigned 25% probability, and it widens the prescriber base beyond hepatologists.
  • Diagnosis rather than eligibility is the commercial bottleneck: 6.2 million Americans have F2-F3 disease but only about 380,000 are currently treatable at a 7.5% diagnosis rate. At a $47,400 annual list price, roughly $30,800 net, peak 2030 revenue is modelled at $1.81B, $3.50B and $6.31B across the bear, base and bull cases.

How it was done

Phase 3 results for resmetirom were set alongside those of the failed NASH candidates and the GLP-1 agonists still in development, drawing on the MAESTRO-NASH trial record, FDA documents and published literature. Approval likelihood was then decomposed into six weighted drivers — safety, unmet medical need, benefit-risk balance, fibrosis efficacy, NASH resolution and trial design — and scored for both resmetirom and obeticholic acid to isolate what separated them. A market funnel worked from NASH prevalence down through fibrosis staging and diagnosis rates to a treatable population, and revenue was projected from 2024 to 2030 under bear, base and bull peak-penetration assumptions of 11%, 20% and 35%. The result was a 19-page brief with ten figures covering the regulatory timeline, safety comparison, market funnel and revenue scenarios.

Limitations

Approval came through the accelerated pathway, so the confirmatory MAESTRO-OUTCOMES trial, estimated to complete between 2026 and 2028, still has to show a reduction in major adverse liver outcomes. The revenue scenarios also depend on penetration and payer assumptions and on how the Phase 3 GLP-1 readouts in NASH land.
